This page will provide information that will help you use the "Service
Provider Search Form".
There are numerous ways to search for providers
using the Service Provider Search Form. The most important thing to
remember is to only complete the criteria essential for your search. In
most of your searches, you will only need to select a couple of items on the
Form.
For example, if you are looking for a list of
Physical Therapists in Lowndes County, for "Type of Service", select
Physical Therapist and for "County Served", select Lowndes.
You can search by Program Name, Type of Service,
Payment Types - Medicaid or Sliding Scale, Geographic Areas, Ages Served,
Language Interpreters Available, and Disabilities Served. (Please
Note: Only use Disabilities Served when you are searching for a specific
support group or organization, such as the Autism Society of America).
If you do not obtain the results expected,
broaden your search criteria. To get the broadest results, for example,
only select a Type of Service and a geographic region or county.
The database search will display up to 100
providers at a time. If your search is too broad, yielding more than 100
providers in the results, you must narrow your search criteria.
Below is a description of each item on the Search
Form.
Type of Service - "Type of
Service" refers to the types of services offered by the service
provider. Examples of categories are "Occupational Therapist"
and "Counseling & Psychological Services." Providers are
categorized in over 100 service categories. Providers that offer multiple
services may be listed under several categories. View
or print the list of available search categories.
Geographic Area - There are 2 ways to
narrow your search geographically if you desire to limit your resources to a specific
area. You can search by Region or County Served.
Region - pulls up providers whose resident address
is in a specific group of counties. We have divided the state into
the following regions:
Albany Region: Baker, Calhoun,
Clay, Colquitt, Crisp, Decatur, Dougherty, Early, Grady, Lee, Miller, Mitchell,
Quitman, Randolph, Seminole, Terrell, Thomas, Worth
Athens Region: Barrow, Clarke,
Elbert, Greene, Jackson, Madison, Morgan, Oconee, Oglethorpe, Walton
Atlanta 5 County Region: Clayton,
Cobb, DeKalb, Fulton, Gwinnett
Atlanta 12 County Region: Cherokee,
Clayton, Cobb, DeKalb, Douglas, Fayette, Forsyth, Fulton, Gwinnett, Henry, Newton,
Rockdale
Augusta Region: Burke, Columbia, Emanuel,
Glascock, Jefferson, Jenkins, Lincoln, McDuffie, Richmond, Screven,
Taliaferro, Warren, Wilkes
Columbus Region: Chattahoochee,
Harris, Marion, Muscogee, Schley, Stewart, Sumter, Talbot, Taylor, Webster
Gainesville Region: Banks,
Dawson, Franklin, Habersham, Hall, Hart, Lumpkin, Rabun, Stephens, Towns, Union,
White
Macon Region: Baldwin, Bibb,
Crawford, Dooly, Hancock, Houston, Jasper, Jones, Lamar, Macon, Monroe, Peach,
Putnam, Twiggs, Upson, Washington, Wilkinson
Savannah Region: Bryan, Bulloch, Candler, Chatham, Effingham, Evans, Liberty, Long, McIntosh, Tattnall, Toombs
County - Pulls up providers serving that
particular county. For
example, if you search for Physical Therapists serving Bartow County, you will
get a list of providers physically located in Bartow, in addition to providers
in neighboring counties that serve Bartow . Your search results may also
include some statewide resources that have indicated they serve all counties.
Disability - In most searches, you should
leave this item blank. Many of the providers serve all types of
disabilities. As there are over 1000 disabilities and conditions, these
providers are not listed under each disorder name. Only complete this item
if you are looking for
a Support Group or Disability Organization that focuses on a
particular disorder. You
have the option of choosing "Contains" or "Starts
With."
Program Name - Enter the name of the
program you are searching for. You may enter a partial name. The
more precise you enter the name, the fewer the matches that will be returned to
you.
You can choose "Contains" or "Starts With"
- If you choose "Contains" from the pull-down box next to
"Program Name", the search will return all matches where the program
name simply contains the words/letters you entered for the program name.
If you choose "Starts With" from the pull-down box, the search will
only find program names that actually start with the words/letters you entered
for the program name.
Payment Types - You can leave it on the
default setting of "Any Payment Type" or you can narrow your search to
only
those providers who accept Medicaid or Sliding Scale payment
Ages Served - You can narrow your search
to include only providers serving a specific age group, such as age "3-5
years". If you select a specific age group, your results will include
providers specializing in that particular age group, in addition to providers
that serve "all ages."
Language Interpreters - Some providers
have indicated that they have access to foreign or sign language interpreters
either on their staff or via an interpreting service. You can narrow your
search to include only those providers who have indicated they have interpreting services for a
specific language.
Results (Output) Format - You have several
choices for how your results will be displayed:
Show Info - You can select
"Detailed Information" or "Brief Information".
"Detailed" includes many details and descriptive information for each
provider. "Brief" output only lists the program name, phone
number, and city.
Display As - You can also display your
results in a Table Format or Text Format.
Sort By - you can choose to sort your
results alphabetically either by Program Name, County or City
Service Providers
Providers Currently Listed - If you are
currently listed in the database and would like to make changes to your record,
click on the link at the bottom of the Search Page for "Add/Update Provider
Information" and complete the form.
Providers Seeking to be Added - If you
would like your service added to the database, click on the link at the bottom
of the Search Page for "Add/Update Provider Information" and complete
the form.
